ความสามารถในการทำงานลดลงพบได้บ่อยในผู้สูงอายุที่เข้ารับการรักษาในโรงพยาบาล มีหลายการศึกษาที่รายงานความสัมพันธ์ของการเกิดความสามารถในการทำงานลดลงและผลลัพธ์อันไม่พึงประสงค์ตามมา เช่น การเพิ่มขึ้นของความพิการ การใช้ทรัพยากรทางการแพทย์ และการถูกส่งไปยังบ้านพักคนชรา ยิ่งไปกว่านั้นการลดลงของความสามารถในการทำงานสัมพันธ์กับอัตราการเสียชีวิตที่เพิ่มขึ้น สาเหตุของความสามารถในการทำงานลดลงในผู้สูงอายุเกิดจากปัญหาหลายด้าน ประกอบไปด้วย ปัญหาทางกายภาพ ปัญหาสมรรถภาพสมองและจิตใจ และปัญหาด้านสังคมและสิ่งแวดล้อม จึงมีคำแนะนำให้ใช้การประเมินผู้สูงอายุแบบรอบด้าน (comprehensive geriatric assessment, CGA) ซึ่งเป็นเครื่องมือสำคัญในการตรวจประเมินผู้สูงอายุที่มีความสามารถในการทำงานลดลง นอกจากนั้นภายหลังจากหาสาเหตุและปัจจัยเสี่ยงของการเกิดความสามารถในการทำงานลดลง ทีมสหสาขาวิชาชีพร่วมกันให้การป้องกันและแก้ไขการเกิดความสามารถในการทำงานลดลง ซึ่งเป็นสิ่งสำคัญที่จะช่วยเพิ่มคุณภาพชีวิตที่ดีให้กับทั้งผู้ป่วยและญาติ ในบทความนี้มุ่งเน้นถึงการประเมินเพื่อหาปัจจัยเสี่ยงและสาเหตุ รวมถึงแนวทางการป้องกันการเกิดความสามารถในการทำงานลดลงของผู้สูงอายุที่เข้ารับการรักษาในโรงพยาบาล

Functional decline occurs very common among hospitalized older patients. Several studies have demonstrated the associations between functional decline and unpleasant consequences such as increased disability, healthcare utilization and nursing home placement. Moreover, the decline in function has also been reported to associate with increased mortality. The causes of functional decline in older persons could stem from several domains including physical, cognitive and psychological, and social and environmental problems. It has been suggested that comprehensive geriatric assessment (CGA) is the important tool for evaluating older people with functional decline. Additionally, providing multidisciplinary team interventions after identifying the causes and risk factors, in order to prevent and correct contributable causes of functional decline is the key element for improving patients and families’ well-being. This review article provides scope for the evaluation for the risk factors and causes and suggest preventive strategies for functional decline in older persons admitted to the hospital.
